Awards Submissions

DEADLINE FRIDAY, JUNE 4, 2021 

Award Categories are listed below.

• Albert G. Richards Award
• Centennial Award
• Howard Raper Award
• Charles Morris Award
• William H Rollins Award

You will be asked to first submit the Awards Application.

Supporting documents will then have to be e-mailed to [email protected] for each award.

• Albert G. Richards Award - Curriculum Vitae, Research Proposal, Letter of recommendation from the OMFR residency program director
• Centennial Award - Curriculum Vitae, Statement of Goals, Three (3) letters of reference - one (1) from OMFR, two (2) of student's choice 
• Howard H. Raper Award - Curriculum Vitae, Career Plans, Letter of reference from the OMFR residency program director
• Charles R. Morris Award - Manuscript and Institutional nomination letter
• William H. Rollins Award - Curriculum Vitae, Research Summary, Letter of recommendation from the program director (for a resident) or Department Chair (for faculty)

All awards require at least one nomination letter or letter of recommendation.  These letters must include candidate's name and award they are applying for in the name/title of the document and be sent directly to [email protected].  Candidates are not to see these letters.
